Summary:
The Chronic Care Manager is a vital member of the healthcare team in a primary care setting.

The Chronic Care Manager collaborates closely with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes and enhance the overall quality of care for patients living with chronic diseases.
Responsibilities:

Care Coordination:
- Identify patients with chronic medical conditions and establish a care plan based on the individual patient's needs and goals.
- Schedule regular follow-up appointments and coordinate with patients to ensure they adhere to their treatment plans.
- Collaborate with healthcare providers to manage the care of patients with complex medical conditions.
- Facilitate communication and information sharing between different members of the healthcare team involved in the patient's care.

Patient Education and Empowerment:
- Educate patients and their families about their chronic conditions, treatment options, and lifestyle modifications to manage their health effectively.

Monitoring and Assessment:

Health Promotion and Prevention:

Qualifications:

- Previous experience in chronic care management, case management, patient advocacy, or LPN or Medical Assistant in a primary care setting is preferred - Strong knowledge of chronic medical conditions, their treatment modalities, and the healthcare system.

- Ability to work collaboratively in a team-oriented environment.
- Proficiency in using electronic health record systems and other healthcare-related software.
