POSITION
Summary: This position is responsible formaintaining the continuity of care and services to Hospice patients, patient's families and other loved ones through the use of a comprehensive Plan of Care established at the time of admission and as periodically updated.
Core Responsibilities Include:
1. Provide Patient Care Services as directed by the Registered Nurse
2. Initiate and Maintain Proper Documentation
3. Interact with and Support Interdisciplinary Team
Other Responsibilities/Functions:
1. Maintain Professional Standards
2. Assist Registered Nurse with Patient Care Services
Requirements
Licensed in the State of Florida as a Registered Nurse
Minimum 1 year experience with background in Hospice or Home Health preferred
CPR certification, IV Therapy
Ability to accept various individual lifestyles
